Please call your insurance carrier to verify if we are a preferred provider for your plan prior to your appointment.

Our practice is contracted with most major PPO health insurance plans.

You are responsible for paying all fees, including co-payments, at the time of your office visit

Office Policy

• $35 No Show Fee will be charged for any missed appointment that was cancelled less than 24 hours prior to the visit. Please be considerate and call in advance if you are not able to come in.
• $25 fee will be charged for any bounced or cancelled checks.
• There is a 48 hour turn around for prescription refills.
• If you have not seen the physician in 3-6 months your prescription will not be refilled.
